Before Your First Visit

After you’ve scheduled your initial appointment, there are a few things you should keep in mind.

Make sure you:

  • Review all documents provided, if any
  • Completely fill out any requested forms or paperwork
  • Schedule at least two hours for your appointment
  • Arrive 15 minutes prior to your appointment time
  • Don’t forget your photo ID, insurance card, and medical records

We know that going through fertility treatment processes comes with a number of questions and concerns. To help us assist you, make sure you come prepared with any questions. You can even write them down if needed! We want to make sure that you are fully prepared so we can work with you to create a plan custom-tailored to meet your unique needs.

Don’t Let Cost be a Barrier to Starting Your Family

Before your appointment, our team will check with your health insurance carrier to see what, if anything, you may be covered for. This is done to help you fully understand any costs you may be liable for before you proceed with your infertility treatments.
