Why IVF Embryo Grades Don't Work Like Report Cards

When you're going through IVF, every update from the lab feels like a cliffhanger. Embryo grades are not report cards, and they certainly don't tell the whole story about your chances.
When you're going through IVF, every update from the lab feels like a cliffhanger. One of the most suspenseful moments? Hearing your embryo's grade. It's tempting to think of embryo grading as a report card: an A equals success, and a C means trouble. But embryo grades are not report cards, and they certainly don't tell the whole story.
What Is Embryo Grading?
Embryo grading is a system embryologists use to describe embryos based on how they look under a microscope. On Day 3, they check the number of cells, fragmentation, and symmetry. By Day 5, embryos are evaluated as blastocysts, where the inner cell mass and the trophectoderm are graded separately.
Why Higher Doesn't Always Mean Better
An embryo with an "AA" grade does not automatically lead to a healthy pregnancy. Many parents have welcomed babies from embryos that looked "average" in the lab. Embryo grading only looks at appearance, not what is happening inside the embryo.
This is why some fertility specialists recommend preimplantation genetic testing (PGT IVF). Unlike grading, which focuses on appearance, genetic testing during IVF can reveal whether an embryo has the correct number of chromosomes.
What Embryo Grading Can and Cannot Do
It can provide a snapshot of embryo development, help embryologists decide which embryos to transfer or freeze, and offer context during your fertility consultation. It cannot predict with certainty whether pregnancy will occur, determine your future child's health, or replace genetic testing.
Common Myths About Embryo Grading
Lower-graded embryos never work? False — many healthy pregnancies begin with "average" embryos. Grading is the same everywhere? Not exactly — while guidelines exist, grading has some subjectivity between clinics.
