Understanding Your Ovarian Reserve
Ovarian reserve testing is a critical assessment that helps determine the quantity and quality of eggs remaining in your ovaries. Ovarian reserve testing evaluates how many eggs you have left and how well they're likely to respond to fertility treatments. This information is invaluable whether you're trying to conceive now, considering fertility preservation, or simply want to understand your fertility timeline.

Types of Ovarian Reserve Tests
Anti-Müllerian Hormone (AMH) Test
AMH is produced by cells in developing egg follicles. Unlike other fertility hormones, AMH levels remain relatively stable throughout your cycle, making it the most reliable single indicator of ovarian reserve. Higher levels indicate more eggs remaining, while lower levels suggest diminished reserve.
Follicle-Stimulating Hormone (FSH) Test
FSH is measured on day 2-4 of your menstrual cycle. When ovarian reserve declines, the body produces more FSH to try to stimulate the ovaries. Elevated FSH levels can indicate decreased ovarian reserve.
Estradiol (E2) Test
Measured alongside FSH on cycle day 2-4, estradiol helps provide a complete picture of ovarian function. Abnormally high or low estradiol levels can affect the interpretation of FSH results.
Antral Follicle Count (AFC)
Using transvaginal ultrasound, we count the number of small follicles visible in your ovaries at the beginning of your cycle. A higher count indicates better ovarian reserve, while a lower count suggests diminished reserve.
Understanding Your Test Results
Normal Ovarian Reserve
AMH above 1.0 ng/mL, FSH below 10 mIU/mL, and adequate antral follicle count indicate good ovarian reserve. You have a good chance of conceiving naturally or with fertility treatment.
Diminished Ovarian Reserve
AMH between 0.5-1.0 ng/mL, FSH 10-15 mIU/mL, or reduced follicle count suggest declining reserve. Fertility treatment may still be successful, but timing is important.
Low Ovarian Reserve
AMH below 0.5 ng/mL, FSH above 15 mIU/mL, or very few follicles indicate significantly reduced reserve. Aggressive treatment or donor eggs may be recommended.
Important: Ovarian reserve testing measures egg quantity, not quality. While age affects both quantity and quality, these tests specifically assess how many eggs remain and how well your ovaries might respond to treatment.
