Platinum IVF Refund Plan

Palm Beach Fertility Center offers the Platinum IVF Refund Plan to eligible self-pay patients pursuing in vitro fertilization (IVF). The program provides a single bundled program fee covering multiple treatment cycles and may include a partial refund of program fees if a qualifying live birth is not achieved, as described in the full patient agreement.
Included in the Program Fee
Retrieval cycles and embryo transfers must be completed within two (2) years of program enrollment. Unused cycles or transfers at expiration are forfeited with no refund.
- Up to three (3) complete IVF egg retrieval cycles (completed oocyte retrieval required).
- All subsequent frozen embryo transfer (FET) cycles using embryos created under the program, up to six (6) transfers, until embryos are exhausted or a qualifying live birth occurs.
- Standard stimulation monitoring (practice-approved lab blood tests and ultrasounds during stimulation).
- Oocyte retrieval procedure.
- Standard fertilization, including intracytoplasmic sperm injection (ICSI).
- Embryo culture and cryopreservation of excess embryos created during the program period.
- Embryo storage for the duration of the two-year program period.
- Assisted embryo hatching, if clinically indicated.
Complete Cycle Definition
- A complete cycle requires a performed egg retrieval, regardless of oocytes retrieved.
- Cycles canceled before trigger due to inadequate response, or retrievals with no oocytes retrieved, do not count against the allotment.
- Cycles with conception but no qualifying live birth do count toward the program cycle allotment.
- All frozen embryos from a retrieval must be transferred before another egg retrieval.
Third-Party Services Not Covered
These remain the patient's responsibility regardless of program outcome.
- Pre-cycle diagnostic testing, labs, and consultations for eligibility.
- All fertility medications (injectable and oral).
- Pre-cycle genetic carrier screening (PGT-A during IVF is addressed under included services).
- Anesthesia for oocyte retrieval.
- Obstetric care after confirmed clinical pregnancy.
- Complication management or hospitalization.
- Embryo cryostorage after program period, termination, or successful outcome.
- Male factor diagnostic or surgical sperm retrieval procedures.
- Services from third parties not affiliated with the practice.
Optional Add-On Services
Available at additional cost; not included in the program fee and not eligible for program refund.
- Surgical sperm retrieval (TESA, TESE, or MESA).
- Estrogen Androgen Priming Protocol (EAPP).
- Ovarian Rejuvenation.
- Platelet-Rich Plasma (PRP) therapy.
- Bioserum Particles.
- Donor sperm from an accredited cryobank.
- Enhanced sperm prep and ZyMot sperm selection.
Program Fees
The program fee is due in full prior to the first treatment cycle. Your financial counselor will provide current program and optional service pricing at enrollment. This website does not publish individual fee amounts.
Eligibility Overview
Enrollment requires meeting all clinical and financial criteria verified before enrollment. The practice may deny or dis-enroll patients who do not meet or maintain eligibility.
- Age 37 or younger at enrollment when using own eggs.
- Own-egg cycles: AMH ≥ 1.5 ng/mL, AFC > 8, and normal ovarian reserve criteria per agreement.
- BMI between 18 and 35 at enrollment, maintained throughout the program.
- Normal uterine anatomy confirmed by SIS within six months prior to enrollment.
- Normal prolactin and TSH; not peri- or post-menopausal per clinical assessment.
- Sperm criteria met per agreement (partner/donor TMC, surgical retrieval, or FDA-compliant donor sperm).
- No more than two (2) prior failed IVF cycles; additional factors evaluated case by case.
- Non-smoking and no tobacco, nicotine, or recreational drug use for six months prior to enrollment.
- Medical and psychological clearance for treatment and pregnancy.
- Self-pay only; active IVF insurance or employer fertility coverage disqualifies enrollment.
- Genetic eligibility criteria per agreement (no disqualifying chromosome or carrier findings).
Outcomes & Refund
Qualifying success
A qualifying live birth completes the program successfully. No refund is issued; unused cycles are forfeited. Remaining embryos may be stored at patient expense under a separate agreement.
Refund eligibility
If all allotted retrieval cycles and embryo transfers are completed within the two-year program period, no qualifying live birth occurs, and the patient remains in full compliance, a written refund request may qualify for a 50% refund of the program fee only (not optional or third-party services).
Patient-initiated termination
Patient-initiated termination or failure to complete the program within two years does not qualify for a refund.
Important Disclosures
- Patients who achieve a live birth on an early cycle may pay more under this program than standard fee-for-service pricing. The program fee reflects bundled coverage across multiple cycles.
- Clinical eligibility criteria select patients with a favorable prognosis; individual success should be compared to program cost before enrolling.
- SART-reported success rates for similar patient profiles are available from your physician and at www.sart.org; they are not a guarantee of individual results.
- The practice retains the program fee on success and may issue a refund on program failure; medical decisions are made on clinical merit per ASRM guidelines.
Patient Obligations
- Attend scheduled monitoring, consultations, and procedures.
- Follow prescribed medication protocols and report non-compliance.
- Maintain BMI, non-smoking, and sobriety requirements throughout the program.
- Notify the practice immediately if IVF insurance coverage is obtained.
- Exhaust program embryos before a refund request; embryos cannot be retained while seeking a refund.
- Use embryos exclusively with Palm Beach Fertility Center.
- Pay optional and third-party service fees when due.
